Cost: $200.00 (initial sign up fee) $125.00 (each month following). You will be billed on the first of each month. You can cancel at any time with a 10-day written notice prior to the first of each month. Cancellations from the 1st of the month to the 20th of the month will apply to the following month (example: if you want to cancel your membership for April you must give us written notice by March 20th. If you send us written notice on April 3rd telling us you want to cancel April that cancellation will apply to May.)

I understand I will be charged monthly until I cancel future sessions with at least a 10 day written notice. I further understand that there is no refund for any missed sessions regardless of the reason. Furthermore, I understand that participation in No Fear Basketball and Washington Girls Select Basketball involves risk and dangers of serious and permanent bodily injury and death. I hereby release, hold harmless, discharge and agree not to sue No Fear Basketball, Washington Girls Select Basketball, Advantage Basketball Camps, Hummel Enterprises Inc., Michael Hummel, its directors, officers, employees, coaches, officials, volunteers, agents, sponsors, advertisers, owners/leasers of premises for all liability from my participation in these and any other related travel, lodging, social/recreational activities.

I also understand No Fear Basketball, Washington Girls Select Basketball, Advantage Basketball Camps and Hummel Enterprises Inc. retains the right to use for publicity and advertising, photographs and video taken of the participants.
